This office provides blank forms to be completed by the business owner, notarized, and then returned for filing.
The filing fee is $26 for the first page and $4 for each additional page per document.
Please read "Where do I file my Assumed Name - Local or State". Not all Assumed Name Certificates are filed at the County level, certain entities are required to file an assumed name certificate with the Secretary of State. Please file accordingly.
